Dr. Bazin began her career in ambulatory care in 2020 when she joined the Ochsner Digital Medicine team after completing her residency. She is a clinical pharmacist responsible for the management of diabetes, hypertension, and dyslipidemia. She is passionate about educating patients and ensuring they have the knowledge and resources needed to pursue healthier lifestyles.

Dr. O’Flynn joined the Ochsner Digital Medicine team in 2019. She has been in ambulatory care practice since 2015. She is responsible for the provision of medication and lifestyle therapy for chronic disease state management through CDTM. She is responsible for optimization of therapeutic regimens including monitoring patient collected data, mitigating adverse events, providing patient education, and evaluating laboratory results. Dr. O’Flynn serves as a preceptor for pharmacy practice residents.

Dr. Chou has been a clinical pharmacist in ambulatory care since 2010 and with the Ochsner Digital Medicine team since 2017. She enjoys working with patients to improve their self-management of chronic disease states utilizing remote patient monitoring, collaborative drug therapy management, and healthy lifestyle education.

Dr. Colosino joined the Ochsner Digital Medicine team in 2017. She is responsible for optimization of therapeutic regimens including monitoring patient collected data, mitigating adverse events, providing patient education, and evaluating laboratory results. Dr. Colosino serves as the Residency Chair for Digital Medicine and has taken residents since joining Ochsner Health in 2010. She is passionate about giving back to her profession through educating residents and training new pharmacists in her role.

Dr. Elsayed joined the Ochsner Digital Medicine team in 2020. She enjoys mentoring residents, students and new staff to meet departmental goals.

Dr. Monson joined the growing Ochsner Digital Medicine team in 2019. She thrives through the relationships formed with patients made in remote chronic disease monitoring and management. While assisting patients in improving their health outcomes through education and optimizing control through medication management, she also enjoys working with patients on becoming better self-advocates of their own health.

Dr Mvere joined the Ochsner Digital Medicine team in 2020. She is a clinical pharmacist who focuses on management of hypertension, diabetes, hyperlipidemia. Prior to joining Ochsner she was an ambulatory care pharmacist at an FQHC in Arkansas, where she helped patients manage several chronic conditions including those mentioned above, asthma, COPD and HIV. She volunteers for the Texas Pharmacy Association Public Policy Council, the Arkansas Association of Health-System Pharmacists New Resident and Student Council and the Arkansas Pharmacists Association Communications Committee.
