Transcript Requests - Allied Health Sciences
Individuals that were enrolled in a previous Ochsner Allied Health Program can request up to two copies of their official transcripts per year at no cost. Official transcripts will be sent to the recipient address indicated on the form via USPS 1st Class Mail. Please allow 7 business days for processing.
Official Transcripts can be acquired by:
- Completing the Transcript Request Form.
- Submitting the Transcript Request Form to ClinicalEducation@ochsner.org
Education Verification can be requested by email at ClinicalEducation@ochsner.org. Please send us the form requested by the third party and all relevant information for your period of attendance, including name, program, and period of attendance. Allow 5-7 days for processing.
