You are encouraged to contact your board analyst for response to licensing timelines and/or questions. Analyst information can be located in your New Innovations checklist.

Important – you will NOT be allowed to begin residency/fellowship training with Ochsner Clinic Foundation until you have been issued one of the following:

  • US Medical Graduate Training Permit (PGY)
  • Foreign Medical Graduation Training Permit (GETP)
  • Fellowship Training Permit

Please contact your program director or program manager if you have questions of what medical license to apply for prior to the start date.

Prior to your start date, you are required to have successfully completed all required online assignment modules: IHI Open School and Ochsner Learning Network - OLN. Noncompliance will result in not being allowed to clear orientation activities, and you will be placed on mandatory vacation until successful completion.

You are required to apply for and obtain an NPI number. Once assigned, this unique identifier will follow you throughout your medical career. The number is permanent and will not change (no matter where you train or practice). You will only be required to update your address via the NPPES (National Plan & Provider Enumeration System) as you move within the United States.

INTERNS - (PGY-1): The following links will assist in the process for new application requests:

PGY-2 and above resident/fellows: You are required to change the Business/Practice Location information from your previous training facility to Ochsner Clinic Foundation.

Frequently asked questions to assist in the completion of a new application:

Am I a Sole Proprietor? – NO

What should I use for my practice and/or business address? – Use an address and phone number where you can be contacted if they need to contact you. Once you begin your residency, this can be updated to your residency program’s address. NOTE: Any contact information (address/phone) entered with the NPI application is publicly available. Use caution and update your registration to remove mobile numbers as soon as you begin your program.

Should I indicate that I have an MD? – Yes, if you have already graduated (fellowship). Otherwise, interns can update information after you graduate to indicate your degree.

Do I need to include other identification numbers?– No, not at this time. You can update your NPI registration after you receive other IDs such as your LA State Medicaid Provider Number.

What is my primary taxonomy? 390200000X – Student in an Organized Health Care Education/Training Program

Complete tax forms in detail following provided instructions and worksheets.

  • W4 – Federal Tax Form
  • L4 – Louisiana State Tax Form
  • I-9 – Employment Eligibility Verification Form

Note: You will be required to bring ONE item from List A OR ONE item from List B AND List C with you the day you report for GME Orientation to verify your I-9 Employment Eligibility Verification form.

Please upload copies of this documentation to your New Innovations On-boarding Checklist, otherwise you will be required to provide the originals at Orientation:

  • ACLS Certification
  • Internship Certificate and/or Residency Certificate
  • Medical School Diploma
  • Resident/Fellow's Association Memo and Enrollment Form (optional - if you would like to decline, please indicate on Enrollment Form)
  • Vehicle Registration Form

I-9 Form - Lists of Acceptable Documents
You are required to bring ONE item from List A OR ONE item from List B AND List C. The Lists of Acceptable Documents are listed on the back of the I-9 form. GME staff will make copies and return originals at the end of the day’s sessions.

Direct Deposit
You will be required to set up direct deposit. Bring bank account and routing information with you to GME sessions.

Employee Health Screenings
Employee health nurses will conduct several health screenings during orientation sessions

  • Drug Screening (UDS)
  • Mask Fitting
  • TB Testing
  • Color Vision Testing

Refer to your personalized schedule for when and where to report. Information will be provided in your Welcome Letter.

Parking

  • Academic Parking Map - Orientation sessions are held in the Academic Building located at - 1401 Jefferson Highway, New Orleans, LA 70121. Parking signs will be posted and visible along Jefferson Highway.
