Eligibility
Graduate from U.S. ACGME accredited orthopedic surgery residency training program and meet the following requirements:
- Obtain a fully unrestricted LA State Medical License from the LSBME
- Obtain LA CDS license from the LA Board of Pharmacy
- Obtain DEA license from the Diversion Control Division
